Quick answer: A process usually won't publish because it has an orphan step or decision point, which is an item that isn't connected to the rest of the process map. Link it into the map or delete it, then publish again.
Find and fix orphan items
1. Open the draft process and go to the Map subtab.
2. Look for any step or decision point that has no connector going in or out of it.
3. Either connect it to the right place in the process, or delete it if it isn't needed. Deleting an orphan item removes only that item, not the rest of your content.
4. Save, then send the process for review and publish it.
Not sure which item is the orphan? Send us the process name and your web address and we'll take a look.
